How many (account, client key) approvals MemoryDecisionStore
remembers for DecisionRecord::first_time.
The set is cleared rather than trimmed when it fills, so every client
looks new again for a while. That is the failing-safe direction: true
tells an agent to look harder at a client it may in fact have seen, and
false would tell it not to bother with one it has not.
